This resource page aims to provide updated Yishun Health related information and resources, to support primary care partners and their patients in the community.

Our Communities of Care

Yishun Health is committed to building of Communities of Care (CoC) with you, our community partners and resident groups. CoC is a network of partners working together to enable community-based care and meet the needs of our residents.

We offer a variety of resources, activities and services based in the community to support residents in managing their health. These are some examples:

Regional Teams

Yishun Health's Regional Teams cover 6 regions in Yishun and Sembawang. Our teams of nurses and connectors provide residents with the skills, tools and links to self-manage and render mutual help to each other in:

  • Checking blood pressure, blood sugar, height & weight
  • Learning about healthy lifestyle choices
  • Building a personalised care and support plan, and signposting to community support to live well with their long-term conditions
  • Addressing questions on medications, risk of falls, memory loss, community resources and programmes

At our Community Health Posts, residents can walk in to meet our Regional Teams, and access the support we offer. These Community Health Posts are located in neighbourhood spaces at Yishun and Sembawang.

Community Health Screening

We organise community health screening at community spaces such as marketplaces, to allow more convenient access to subsidised screenings.

To facilitate clinical follow-up for individuals at risk, we partner primary care practitioner(s) in the area to provide post-screening consultation for residents.

UP Series

Our UP Series of programmes offers residents the opportunity to take action and experience activities that promote health and wellbeing. Some examples include healthier eating, singing for lung health, and dances for better posture and stability.

Wellness Kampung

Wellness Kampung is a network of three wellness and care centres located in Yishun.

At the wellness centres, residents can learn about healthy living and interact with each other to form a wider care-support network.

Be a part of this Community of Care!

Together with our community partners, resident groups and YH, you can be a part of our community of care in the North to support our residents! You can support patients and residents in gaining access to community resources through the following platforms.

Residents able to self-help

Healthy 365 App

Using this mobile app, residents can track their physical activity and diet, and search for fitness programmes in their community.

This app can be downloaded onto mobile devices via:

NHG Cares App

​From this mobile app, residents can access their medical records, medications and medical certificates, and use digital services such as appointment booking, queue enquiry and medication refill.

Through the Health Kampung section of the app, residents can also access health screening information, active ageing materials and social programmes. They can also view and use their NHG Cares membership benefits.

At the Community Resources section of this website, residents can access the list of the resources offered by Yishun Health, and enquire and sign up directly.
