What is a medical report?
A medical report is a structured document, usually in a standard form provided by an insurance company or relevant authority. It contains medical information that must be completed by an assigned doctor.
What documents can I submit for my outpatient visit or hospitalisation claim?
For most insurance claims, you may submit:
- Visit Summary (for Accident & Emergency visits and 23-hour stays, and/or
- Inpatient Discharge Summary (for inpatient admissions).
If your insurer requires additional information, they may provide a specific medical insurance form for completion.
Why is there a charge for a medical report?
Preparation of a medical report requires the doctor to review the patient’s complete medical records and provide clinical opinions as requested. This work is performed separately from the patient’s outpatient consultation or hospital stay. As such, consultation fees and hospitalisation bills do not include the cost of medical reports. The fee charged covers the hospital’s administrative processing and the doctor’s professional time and input.
