Heart failure is a progressive condition when the heart muscle is either too weak to pump enough blood to the rest of the body, or too stiff to fill properly. One in five people over the age of 40 will develop heart failure in their lifetime.
Patients who survive acute cardiac conditions often progress to chronic heart failure. Re-hospitalisation frequencies are high and survival rates low, compared to most cancers. One in four heart failure patients are re-admitted within 30 days after discharge. One in four die within a year. Although it cannot be cured, it can be treated with strategies to improve symptoms.
We specialise in advanced heart failure therapy, with expertise in diagnostic testing, medical management, community follow-up and psycho-social support for heart failure patients. Our multi-disciplinary team comprises doctors, specialist nurses, cardiac pharmacists, case managers, physiotherapists, occupational therapists, dietitians and medical social workers. Together, we work to provide individualised care and empower patients to be proactive in managing their own health.
We provide state of the art evidence-based medical therapy. Close follow-up ensures optimisation of medications to bring about the benefits of these drugs in the treatment of our patients.
Some patients with severe heart failure or serious arrhythmias (irregular heartbeat) may require Implantable Cardioverter Defibrillators (ICDs) or Cardiac Resynchronisation Therapy (CRT).
We encourage our patients to join our rehabilitation programme based on their exercise tolerance levels. Studies have shown improvements in patients’ functional abilities, quality of life, re-hospitalisations and survival rates.
