I am an inpatient pharmacist. I am part of a multidisciplinary team caring for patients in the inpatient wards and ICUs. My role as part of the team involves ensuring that all medications prescribed and administered are safe and efficacious for the patients. I also help patients better understand their medications by providing drug education and counselling during admission and on discharge.
What I enjoy most about my job is “helping patients help themselves” through simplifying their complex drug regimens or switching them to more appropriate dosage forms so that their drug regimens are more manageable and befitting of their lifestyles.
I am part of an amazing team of big brains and hearts that run the Cardiology Pharmacist Clinic (CPC) service in KTPH. The individuals that came before me have established an unrelenting reputation for patient-centred care and evidence-based medicine.
Cardiologists refer patients to our service, for assistance with monitoring lab parameters and medication optimisation. A key aspect of our work involves engaging patients in a shared decision making process, offering personalised guidance with the ultimate goal of reducing their cardiovascular risks and improving quality of life.

As an Anticoagulant Clinic (AC) Pharmacist, I meet patients who are newly started on warfarin and those who are stable taking the medication. Through detailed counselling and interviews I identify factors influencing patients' compliance as well as treatment effects.
Involving patients in decision making and observing the outcome definitely motivate me to provide better care for them in my daily work.
I am part of the inpatient team at KTPH Pharmacy caring for patients in the general wards and ICUs. As an inpatient pharmacist, I work together with a multidisciplinary team to care for warded patients. My role as an inpatient pharmacist is to ensure that the medications served in the wards are safe and effective for my patients.
My passion in infectious diseases has led me to be part of the Antimicrobial Stewardship Programme (ASP) team. As a team, we promote and educate healthcare professionals regarding optimal use of antibiotics to prevent emerging multi-drug resistant bacteria or "superbugs".
As a medication safety officer, I step in to resolve situations when unfortunate medication errors occur. I conduct investigations to identify lapses in the systems and processes, and find novel ways to rectify them.
Beyond that, I am motivated to bring about changes to the culture and raise awareness of medication safety amongst my colleagues. It has never been an easy journey. But for medication and patient safety, I believe that every additional step that I take in my daily routine, will go a really long way.
