Overview

We partner with GPs, Polyclinics and Family Medicine Clinics to support patients in gaining access to care for common acute ailments and management of long-term conditions. We strive to achieve this by ensuring that every patient has his/her own primary care physician.

This contributes to the integration of patient care, to support patients as they transit from community to hospital, and from hospital to community.

As part of the national Healthier SG plan, we also partner with GPs to support patients in achieving their health goals and connecting them to community resources (e.g. services, programmes and activities). Find out more at Collaborations with GPs and Resources and Support for GPs.

At NHG, we have reorganised ourselves to serve you better. The Primary Care Office at our Population Health Campus will now support you in Healthier SG, and in partnerships and collaborations with community partners and the hospitals in your neighbourhood.
