Knowing how our lifestyle habits impact our health is important. But even more important is actually living a healthy lifestyle. CHAMP aims to encourage every individual to take charge of their own health by adopting good lifestyle habits.

Whether you are healthy or have a chronic illness, you can lower your risks or improve your condition by making lifestyle changes. Our team of Health Action Coaches will be on hand to give personalised guidance and check on your progress. So everyone can be a CHAMP!

Who can join?

Singaporeans and PRs above the age of 21.

How long is the programme?

You have up to one year to attend talks, group coaching sessions and fitness classes, held in English and Mandarin.

Are there any weekend sessions?

Currently our sessions are all held on weekdays. We may have weekend sessions in the future. 

Where is it held?

Due to the ongoing COVID-19 situation, the sessions have been moved onto online platforms.

The usual face-to-face sessions are held at Khoo Teck Puat Hospital.

Do I have to pay?

The programme is fully funded by MOH; hence, participants aren't required to pay.

Many chronic diseases often go undetected in their early stages with no symptoms and you still feel completely well. Regular screening can help you detect these chronic diseases earlier before they progress, reducing the complications of the chronic disease and easing your downstream financial strain. Finding out your medical condition earlier allows for more timely intervention, either medically or by changing to a healthy lifestyle. Get yourself screened even if you feel healthy!

  • Be a Singaporean aged 40 years old and above
  • Have no known chronic disease at the time of screening
  • Have not done health screening in the last 3 years

  • Green CHAS/No CHAS - $5

Our eyes, ears and mouth are responsible for some of the most important functions in our daily life. As we age, some of these functions will start to decline. Safeguarding our vision, hearing and oral health is, therefore, essential to our quality of life.

Early detection of functional decline with appropriate treatment and lifestyle changes, will enable us to remain independent and active.

Skill UP is a programme that introduces simple lifestyle practices to incorporate in your daily life for a healthier you!

Our series of interactive activities and sharing sessions will equip you with essential skills and knowledge, and kickstart your journey to good health.

In light of the COVID-19 situation, we have adapted our physical session to bring you an online experience. Join us for an experiential learning experience in the comfort of your home.

  • 3 interactive sessions (1.5h per session)

  • Limited to 50 participants for each intake

Tune UP is a community singing programme for persons who experience breathlessness*.

* Tune UP is a not a replacement for pulmonary rehabilitation.

How does singing help you?

  • Breathe more slowly and deeply
  • Improve your posture to ease your breathing
  • Strengthen your core muscles to improve mobility
  • Gain better control of your breathing to reduce anxiety

Tune UP is suitable for adults with the conditions such as:

  • Chronic Obstructive Pulmonary Disease (COPD)
  • Asthma
  • Heart failure
  • Post-Covid condition
  • Chronic smokers

Please commit to the first 12 sessions of the programme, so as to give yourself enough time to truly experience the benefits of singing!

Who are the instructors?

You will be guided by singing instructors certified by the British Lung Foundation Singing for Lung Health programme.

What language is this conducted in?

Sessions are available in English, Mandarin and Malay.
