Knowing how our lifestyle habits impact our health is important. But even more important is actually living a healthy lifestyle. CHAMP aims to encourage every individual to take charge of their own health by adopting good lifestyle habits.
Whether you are healthy or have a chronic illness, you can lower your risks or improve your condition by making lifestyle changes. Our team of Health Action Coaches led by a clinician will be on hand to give personalised guidance and check on your progress. So everyone can be a CHAMP!
Who can join?
Singaporeans and PRs above the age of 40.
How long is the programme?
You have up to one year to attend talks, group coaching sessions and fitness classes, held in English and Mandarin.
Are there any weekend sessions?
Currently our sessions are all held on weekdays. We may have weekend sessions in the future.
Where is it held?
Due to the ongoing COVID-19 situation, the sessions have been moved onto online platforms.
The usual face-to-face sessions are held at Khoo Teck Puat Hospital.
Do I have to pay?
The programme is fully funded by MOH; hence, participants aren't required to pay.

Many chronic diseases often go undetected in their early stages with no symptoms and you still feel completely well. Regular screening can help you detect these chronic diseases earlier before they progress, reducing the complications of the chronic disease and easing your downstream financial strain. Our eyes, ears and mouth are responsible for some of the most important functions in our daily life. As we age, some of these functions will start to decline. Safeguarding our vision, hearing and oral health is, therefore, essential to our quality of life.
Early detection of functional decline with appropriate treatment and lifestyle changes, will enable us to remain independent and active.
To be eligible, you must be a Singaporean aged 60 years old and above.

This is a community exercise programme where you will experience the importance of physical activity, delivered through a series of fun and enjoyable exercises. Together, we can take a holistic approach towards improving daily activities and delay early onset of physical frailty,
