Coaching For Health Action Management Programme (CHAMP)

Knowing how our lifestyle habits impact our health is important. But even more important is actually living a healthy lifestyle. CHAMP aims to encourage every individual to take charge of their own health by adopting good lifestyle habits.

Whether you are healthy or have a chronic illness, you can lower your risks or improve your condition by making lifestyle changes. Our team of Health Action Coaches will be on hand to give personalised guidance and check on your progress. So everyone can be a CHAMP!

Who can join?

Singaporeans and PRs above the age of 21.

How long is the programme?

You have up to one year to attend talks, group coaching sessions and fitness classes, held in English and Mandarin.

Are there any weekend sessions?

Currently our sessions are all held on weekdays. We may have weekend sessions in the future. 

Where is it held?

Due to the ongoing COVID-19 situation, the sessions have been moved onto online platforms.

The usual face-to-face sessions are held at Khoo Teck Puat Hospital.

Do I have to pay?

The programme is fully funded by MOH; hence, participants aren't required to pay.

Community Screening

Many chronic diseases often go undetected in their early stages with no symptoms and you still feel completely well. Regular screening can help you detect these chronic diseases earlier before they progress, reducing the complications of the chronic disease and easing your downstream financial strain. Finding out your medical condition earlier allows for more timely intervention, either medically or by changing to a healthy lifestyle. Get yourself screened even if you feel healthy!

The screening covers:

  • Diabetes
  • Hypertension
  • High cholesterol
  • Obesity

To be eligible, you must

  • Be a Singaporean aged 40 years old and above
  • Have no known chronic disease at the time of screening
  • Have not done health screening in the last 3 years

  • Green CHAS/No CHAS - $5

Functional Screening

Our eyes, ears and mouth are responsible for some of the most important functions in our daily life. As we age, some of these functions will start to decline. Safeguarding our vision, hearing and oral health is, therefore, essential to our quality of life.

Early detection of functional decline with appropriate treatment and lifestyle changes, will enable us to remain independent and active.

For first level functional screening:

Skills For Life  

Notice: Skills for Life has been renamed to Skill UP from 29 April 2021 onwards.

Skill UP is a programme that introduces simple lifestyle practices to incorporate in your daily life for a healthier you!

Our series of interactive activities and sharing sessions will equip you with essential skills and knowledge, and kickstart your journey to good health.

In light of the COVID-19 situation, we have adapted our physical session to bring you an online experience. Join us for an experiential learning experience in the comfort of your home.

We are now accepting sign-ups for our new virtual programme!

Programme structure

  • 3 interactive sessions (1.5h per session)
  • Sessions are conducted over Zoom
  • Limited to 50 participants for each intake
  • For the full experience, please ensure you are able to commit to all 3 sessions

Tune UP

Tune UP is a community singing programme for persons who experience breathlessness*.

* Tune UP is a not a replacement for pulmonary rehabilitation.

How does singing help you?

  • Breathe more slowly and deeply
  • Improve your posture to ease your breathing
  • Strengthen your core muscles to improve mobility
  • Gain better control of your breathing to reduce anxiety

You will be guided by singing instructors certified by the British Lung Foundation Singing for Lung Health programme.

Who can join?

All are welcome, but the movements are gentle and slow to cater to frailer individuals. To reap the most benefits from the programme, individuals should be able to walk at least short distances unaided.

Who are the dance leaders?

The dance leaders are our fellow resident neighbours, who have voluntarily undergone training by professional dance artistes in order to lead the sessions.

What does each session include?

Each session comprises of warm up standing exercises, floor (or modified chair) exercises, and a creative dance component.

Can I sign up to be a dance leader?

Of course, we would love to have you! We are always on the lookout for new dance leaders. Don’t worry, no experience is required. I’m a community partner, can I start Dance UP at my organisation’s site?

If your organisation is located in the Yishun and Sembawang region, we would love to work with you to start a Dance UP group at your site to reach out to more residents in the community.
