4 Transitional Care Service


The Transitional Care (TC) Service by KTPH is a charged service offering personalised post-discharge care management by a team of doctors, nurses and therapists visiting patients with complex and chronic medical conditions. The service is available up to a period of three months (depending on patient’s condition).

The TC team helps patients to transit smoothly from the hospital to their homes to help them achieve an enhanced quality of life through comprehensive clinical treatment and the prevention of medical complications.

The TC team helps patients to transit smoothly from the hospital to their homes to help them achieve an enhanced quality of life through comprehensive clinical treatment and the prevention of medical complications.
 


Services under Transitional Care provided by a dedicated multidisciplinary team include:

Services by TC doctor

  • Review patients with chronic medical conditions such as heart failure, respiratory problems and diabetes
  • Review elderly patients suffering from frequent falls, dementia or incontinence
  • Review post-operative patients with mobility problems

Services by TC nurse

  • Conduct physical assessment
  • Perform nursing procedures (E.g. change of feeding tube, urinary catheter, wound dressing, care of tracheostomy/stoma, etc.)
  • Educate family members on patient’s condition
  • Provide caregiver training

 

Services by Physiotherapist

  • Conduct physical assessment and discuss treatment plan and duration with the individual and caregivers
  • Identify physical limitations and prescribe exercises that will improve patient’s mobility

 

Services by Occupational Therapist

  • Provide patients and caregivers with skills to cope with daily activities
  • Recommend activities at home to improve quality of life
  • Assess home environment and recommend modifications to enhance function and safety

 

Services by Speech Therapist

  • Address communication difficulties in the areas of speech, fluency, voice and language - comprehension and expression
  • Assess the swallowing function, and provide swallowing intervention
