Shared Care Partnership

We partner with GPs, Polyclinics and Family Medicine Clinics to support patients in gaining access to care for common acute ailments and management of long term conditions. We strive to achieve this by ensuring that every patient has their own primary-care physician.

This contributes to the integration of patient care, to support patients as they transit from community to hospital, and from hospital to community.

The Diabetes Remission Shared Care Collaborative aims to reverse diabetes in overweight persons living with early diabetes (within 6 years of diagnosis) to improve their health and wellbeing.

Based on findings from the Diabetes UK-funded Diabetes Remission Clinical Trial (DiRECT) study, diabetes remission is strongly linked to weight loss.

We hence aim to support patients in achieving diabetes remission through a structured weight loss regime (approx. 2 years) which includes:

  • Total diet replacement with a low-calorie meal replacement product
  • Gradual food reintroduction
  • Weight maintenance
  • Regular consultations & reviews with GPs and YH multidisciplinary care team (e.g. physicians, community nurses, dietitians)

Through this collaborative, Yishun Health aims to collaborate with Primary Care Practitioners in the co-management of patients aiming to achieve diabetes remission. Under shared care programmes, Yishun Health Specialists and General Practitioners (GPs) co-manage patients with stable conditions, who alternate their visits between SOC and GP clinics.

Through the collaboration:

  • Patients receive holistic comprehensive care
  • Patients are right-sited to Primary Care Partners
  • GPs get direct access to YH specialists
